Sildenafil Citrate is one of the safest and most widely prescribed treatments used to treat erectile dysfunction (ED). But like any medication, Sildenafil can cause side effects. While most side effects are mild and temporary, some can be uncomfortable and require medical attention.

Side effects are more frequent and pronounced at higher doses, with Sildenafil 100 mg being the highest.

Headaches and flushing

Headaches are one of the most common Sildenafil side effects [4]. Viagra is specifically more associated with headaches. This distinguishes it when compared tadalafil vs sildenafil, for example, as tadalafil is more commonly associated with back pain.

They occur because Sildenafil works by causing the blood vessels to widen, increasing blood flow and affecting pressure within blood vessels in the head.

Facial flushing is also common and may cause:

Symptoms such as dizziness and light-headedness are more likely to occur in older adults and people taking blood pressure medications. If you feel dizzy after taking Sildenafil, it is recommended to sit or lie down until your symptoms improve.

Visual disturbances: blurred vision and colour tinge

Infrequently, individuals may experience a prolonged erection lasting longer than four hours, known as priapism [12]. Priapism is considered a medical emergency as prolonged erections can damage the penis tissue permanently.

Seek immediate medical attention if you have any of the following:

  • An erection lasts more than four hours
  • A painful erection
  • Symptoms do not improve after sexual activity ends

Individuals predisposed to priapism, such as those with sickle cell anaemia, leukaemia, multiple myeloma, or anatomical deformations of the penis (such as Peyronie's disease), must use Sildenafil with extreme caution [12].

If an erection persists past the four-hour mark, immediate emergency medical intervention is vital to prevent ischemic tissue damage, irreversible corporate fibrosis, and permanent erectile dysfunction.

Sudden vision loss and when to seek help immediately

Very rarely, Sildenafil has been associated with sudden loss of vision. 

This may be linked to a condition called non-arteritic anterior ischaemic optic neuropathy (NAION). The exact mechanism is not fully established, but it is thought to involve reduced blood flow to the optic nerve in men with pre-existing anatomical risk factors such as a crowded optic disc [13].

If you experience vision problems, such as partial blindness or significant visual changes that do not resolve, stop taking Sildenafil and seek urgent medical attention.

Sudden hearing loss

Although uncommon, some patients have reported hearing problems as a result of the medication.

Side effects may include [11]:

  • Sudden hearing loss
  • Reduced hearing
  • Ringing in the ears
  • Dizziness accompanying hearing changes

Any sudden hearing changes should be treated as a medical emergency and individuals are advised to contact a doctor immediately.

Severe allergic reactions

Men with cardiovascular conditions

Men with heart disease require careful assessment before using Sildenafil. While many patients with stable cardiovascular disease can safely take Sildenafil, the medication may not be suitable for everyone [3].

Caution is needed in patients with a medical history involving cardiovascular disease, such as a recent heart attack or stroke, particularly if you are currently taking medication.

Kidney and liver problems

The liver and kidneys help process and eliminate Sildenafil from the body [15]. This means it should be taken with caution if you have a history of kidney and liver problems [15]. Patients with severe renal impairment (creatinine clearance less than 30 mL/min) or hepatic insufficiency experience a significant reduction in drug clearance.

Nitrates and Sildenafil: a dangerous combination for drug interactions

Nitrate medications should never be combined with Sildenafil [3].

  • Glyceryl trinitrate (GTN)
  • Isosorbide mononitrate
  • Isosorbide dinitrate

Using Sildenafil while taking nitrates, including amyl nitrate or amyl nitrite, can cause a dangerous fall in blood pressure that may lead to life-threatening complications. If a patient develops acute chest pain or symptoms resembling an acute coronary event after taking Sildenafil, the use of organic nitrates is strictly prohibited within a 24-hour window from the last dose.

Emergency medical services must be immediately notified of sildenafil use to guarantee that safe, alternative non-nitrate cardiac protocols are deployed.

Alpha blockers and blood pressure medications

Certain alpha blockers used for enlarged prostate symptoms or hypertension can also increase the risk of low blood pressure when taken with Sildenafil [3]. To minimize the risk of a sudden drop in blood pressure (orthostatic hypotension), patients must be hemodynamically stable on their alpha-blocker therapy before initiating the medication.

If you experience excessive side effects on the 50 mg dose, a healthcare professional may recommend reducing your dose to 25 mg [5]. Furthermore, patients should be advised to try the medication under the correct conditions on at least 4 to 8 separate occasions before requesting a dose increase or concluding that the treatment has failed, as anxiety and timing can heavily mask initial efficacy [3].

Avoid alcohol and heavy meals

Heavy, fatty meals are among the most common reasons patients experience side effects or reduced effectiveness when taking Sildenafil [3]. Limit alcohol consumption and avoid high-fat meals before taking Sildenafil to improve both safety and performance.

Follow your prescription and never exceed one dose per day
