Why does Viagra require a prescription?

Sildenafil, the active component of Viagra, works by inhibiting the PDE5 enzyme, which otherwise breaks down the chemicals responsible for blood flow to the penis. The same action that boosts blood flow to induce an erection can be dangerous for some men: those on nitrates as a result of heart disease, those with high blood pressure or those on alpha-blockers. That combination can cause a sudden drop in blood pressure, which is a medical emergency.

Prescription oversight ensures a clinician screens for those risks before you take Viagra. It’s also important because erection problems are a classic marker of cardiovascular disease [7]. A thorough medical evaluation will also uncover underlying conditions such as hypertension, diabetes, and hypogonadism (low testosterone), which require comprehensive management rather than just a temporary fix for the symptom.

Can you buy Viagra over the counter?

Yes!  Viagra Connect is available over the counter at any registered UK pharmacy, in-store or online, without a prescription. The sale is not unconditional; the pharmacist needs to make a structured consultation to verify that the medication is safe and suitable for you before they dispense it.

What is Viagra Connect and how does it work?

Viagra Connect is a branded over-the-counter formulation of sildenafil 50 mg made by Viatris. It contains sildenafil citrate, the same active ingredient as prescription Viagra, and works the same way: it inhibits PDE5 to increase blood flow to the penis during sexual activity. It doesn’t cause a spontaneous erection; sexual stimulation is still needed to work. The recommended dose is one Viagra Connect 50 mg tablet taken 30 to 60 minutes before sex, ideally on an empty stomach or after a light meal. Consuming a heavy or high-fat meal slows absorption and delays how quickly the medication takes effect. You should never take Viagra more than once in a 24-hour period.

During your consultation (whether in-store or online), the pharmacist will ask about your blood pressure, cardiovascular health, renal and hepatic function, and any concurrent medications. For instance, if you take nitrates for chest pain (such as glyceryl trinitrate or GTN spray), sildenafil is absolutely contraindicated. The pharmacist will also discuss alcohol use, as combining it with sildenafil can further lower blood pressure and decrease the drug's effectiveness.

Viagra Connect vs prescription Viagra: what's the difference?

The same active ingredients are present in Viagra Connect and prescription Viagra. 

The difference is dose range, clinical review and cost:

Dose: Viagra Connect is a set dose of 50 mg only. Prescription Viagra is available in 25 mg, 50 mg and 100 mg, so your doctor can change the dosage if 50 mg is not enough or causes side effects. 

Oversight: Prescription Viagra requires a clinician review, and Viagra Connect requires a pharmacist review. A doctor can investigate the cause of erectile dysfunction, which a pharmacist cannot. 

Cost: Viagra Connect is generally £5-£20 for a pack of four tablets. Generic sildenafil on prescription is usually cheaper, especially on NHS prescriptions for qualifying conditions [6]. 

Branded version: Prescription Viagra is the original branded version manufactured by Pfizer; Viagra Connect is its consumer-licensed offshoot. Both contain sildenafil citrate. 

Who it suits: Viagra Connect suits men with simple erection problems and no complex health problems. Prescription Viagra (or generic sildenafil) suits men who must be treated and require close monitoring.

The MHRA reclassification covers the dose and indication, not just the brand name. Generic sildenafil 50 mg is pharmacologically the same as Viagra Connect, with the same active ingredient, sildenafil citrate, at the same dose and usually cheaper.

The pharmacist consultation requirements are the same regardless of which brand you choose. And if you need a different dose, 100 mg because 50 mg is not sufficient for erectile dysfunction, or 25 mg because side effects are a concern, then you will require a prescription. Sildenafil 25 mg and 100 mg remain prescription-only.

Are there safe ED treatments available over the counter?

Sildenafil (over the counter) 50mg and Cialis Together 10mg (tadalafil) are the only licensed oral erectile dysfunction medications currently available without a prescription in the UK. All other ED treatments require a prescription.

Cialis (tadalafil): prescription only, is available in 2.5 mg, 5 mg, 10 mg and 20 mg dosages with a daily low dose.

Vardenafil (Levitra and generics): prescription only, is a PDE5 inhibitor with slightly different side effects from sildenafil.

Avanafil (Spedra): prescription only, quick-acting with onset as little as 15 minutes.

Products marketed as natural alternatives to Viagra with L-arginine, ginseng, or maca are not licensed medicines. Many 'herbal' products sold online have been found by the MHRA to contain undeclared sildenafil at uncontrolled quantities, which is illegal and potentially very dangerous. They are not suitable alternatives to licensed erectile dysfunction treatment.

Lifestyle changes (e.g., losing weight, smoking cessation, and increasing exercise) are evidence-based strategies for managing erectile dysfunction, particularly given its strong association with cardiovascular risk factors. These are complementary to medication and may reduce the required dosage over time.

Vacuum erection devices (VEDs) are a non-pharmacological option that is available without a prescription. They are recommended in the NHS and British Society for Sexual Medicine guidelines as a good option for men who cannot or prefer not to take oral erectile dysfunction medication. Sometimes Viagra Connect and VEDs can be used in combination under medical guidance.

What are the health risks of taking Viagra without seeing a doctor?

The reclassification of Viagra Connect is a regulatory judgment that most men can safely use 50 mg sildenafil without a prescription, provided a pharmacist screens them first [4]. That does not mean the medicine is without risk. Men who skip the consultation or buy sildenafil from unscreened sources forgo safety checks for sound clinical reasons.

Side effects

The most common side effects of Viagra are headache (in about 10% of users), facial flushing, nasal congestion, indigestion, and transient visual changes such as a blue tinge or increased light sensitivity. These are usually mild and resolve as the pill leaves your system. Alcohol can worsen flushing and lower blood pressure; the two are best kept separate.

Serious side effects are rare but can include sudden hearing loss, non-arteritic ischaemic optic neuropathy (sudden vision loss), and priapism (a painful erection lasting more than four hours). Priapism is a medical emergency; if left untreated, it can cause permanent erectile tissue damage. Most men can tolerate sildenafil well. If you experience chest pain, severe dizziness, or breathing difficulties after taking a dose, seek immediate medical attention.

Drug interactions

The most dangerous drug interaction is with organic nitrates (e.g., glyceryl trinitrate (GTN) sprays and patches for angina, isosorbide mononitrate and isosorbide dinitrate) and riociguat. And taking sildenafil with these drugs can lead to severe and potentially fatal low blood pressure. This is an absolute contraindication to avoid, and there is no safe alternative. Alpha-blockers for high blood pressure or benign prostatic hyperplasia can also lower blood pressure dangerously with sildenafil [5].

Some HIV protease inhibitors greatly increase plasma levels of sildenafil; these compounds have negative effects that need to be reduced and dosed up. Grapefruit juice inhibits the enzyme that metabolises sildenafil and should be avoided on the day you take it. These interactions are why a medical assessment matters - not to create barriers, but because the drug interaction profile of sildenafil is clinically significant for men on some common medicines. The effect on blood pressure, in particular, can be severe.

How to get Viagra legally in the UK

Through your GP or NHS

For ED without a qualifying condition, NHS availability is dependent on your local Integrated Care Board, and many men pay privately.

This is a way of addressing men who don’t want to deal with the issue face-to-face or need a quick turnaround when an NHS appointment is a good option.

Good online doctor services will turn down any request for safety reasons, as a GP or pharmacist would. You can also purchase Viagra Connect or generic sildenafil 50 mg directly from a registered online pharmacy without a full online doctor consultation, provided the pharmacy conducts its own pharmacist-led screening before dispensing.
