There is no single outcome for everyone, but research shows that some regain is common after stopping treatment. [1, 2]

If you are considering stopping weight loss injections, the safest plan depends on why you are stopping, whether you also have diabetes, and which medicine you use. People stop for many reasons. Common reasons for stopping weight loss injections include reaching a target weight, side effects, limited results, cost and personal preference.

Reaching a weight-loss goal

Reaching your target weight or weight loss goal can feel like the natural point to stop. Some people may no longer need to lose weight at the same pace. However, obesity is a long-term condition, and treatment may also be helping to regulate appetite, reduce food cravings or support blood sugar and blood pressure.

That does not mean everyone needs ongoing treatment indefinitely. It means the decision should consider long term success, metabolic health and the risk of weight regain, not only the number on the scales. A qualified healthcare professional can help decide whether continued treatment, a maintenance approach or discontinuing treatment is appropriate.

Side effects or medical reasons

Side effects are a common reason for stopping weight loss injections. Nausea, vomiting, diarrhoea and constipation are common with Wegovy, Mounjaro and Saxenda, especially during dose escalation. Medical reasons can also make stopping necessary. If acute pancreatitis is suspected, current product information says treatment should be discontinued; if pancreatitis is confirmed, the medicine should not be restarted. Pregnancy is another reason these medicines need to be stopped.

Cost, limited results or personal preference

For weight loss injections, cost, limited results or personal preference can all lead someone to stop. A weight loss treatment may also be reviewed if it is not producing sufficient benefit. NICE advises reconsidering stopping semaglutide if less than 5% of initial weight has been lost after 6 months of treatment. For tirzepatide, continuation should be reconsidered if less than 5% has been lost after 6 months on the highest tolerated dose. Saxenda has its own stopping rule after 12 weeks on the 3.0 mg dose. Stopping because of limited results is not the same as treatment failure. The aim is to choose a weight management strategy whose benefits justify the burden, cost and risks for the individual.

Temporary supply problems can also interrupt treatment. After a prolonged gap, do not automatically restart at your previous dose; the appropriate restart schedule depends on the medicine and length of interruption.

What happens when you stop weight loss injections

As weight loss injections clear from the body, the biological effects that helped you lose weight gradually weaken. Appetite may increase, the medicine's effect on gastric emptying diminishes, and weight regain becomes more likely. The timing varies because semaglutide, tirzepatide and liraglutide remain in the body for different lengths of time.

Appetite, hunger and satiety

Weight loss injections help regulate appetite. The treatment promotes satiety, meaning it can help you feel fuller after eating. When the appetite suppressing effects fade, appetite returns and hunger signals can feel stronger. Some people also notice increased hunger or more food cravings.

This does not mean everyone will suddenly gain weight. However, if hunger rises while eating habits return to previous patterns, it becomes easier to consume more energy than is needed. During treatment, many patients eat fewer calories because they feel full sooner; after stopping, that effect may be less noticeable.

Metabolic and hormonal changes

After weight loss injections are stopped, their direct GLP-1, or GLP-1/GIP, effects fade. For people who also use the medicine to manage type 2 diabetes, blood sugar may rise, which is why stopping needs medical supervision. [8, 9]

Weight loss itself also changes energy needs. A smaller body generally needs less energy, and metabolic rate can fall after weight reduction. Some people experience metabolic adaptation, where energy expenditure drops more than would be expected from the weight change alone. Resting metabolic rate may therefore be lower than before weight loss. [10] This biology can make maintenance harder, but it should not be described as the metabolism “breaking”.

Cardiometabolic improvements may also lessen as weight is regained. In the STEP 1 extension, improvements in blood pressure and other cardiometabolic measures moved back towards baseline after semaglutide withdrawal. [11] A 2026 systematic review likewise found that blood pressure, glucose and lipid improvements tended to reverse after weight loss medication was stopped.

Weight regain after stopping treatment: why you may regain weight

Weight regain after weight loss injections is common, although the amount varies. Some people regain weight quickly and others more slowly. A significant amount of lost weight can return, but the pattern is individual. Evidence on stopping treatment weight regain patterns shows considerable variation between people.

In the STEP 1 extension, participants who had taken semaglutide 2.4 mg regained about two-thirds of their previous weight loss during the year after withdrawal. In SURMOUNT-4, people who switched from tirzepatide to placebo had a mean 14% increase in body weight from week 36 to week 88, while those who continued tirzepatide lost more weight. [12]

Across 37 studies, a 2026 systematic review found average weight regain of about 0.4 kg per month after weight management medicines were stopped. These clinical trials and pooled estimates describe groups, not what will happen to every individual.

Weight regain reflects biological pressures as well as behaviour. It is one reason weight management often needs a longer-term plan.

Medical guidance is especially important if the injection also helps control diabetes, because discontinuation can affect blood sugar as well as appetite and weight.

Gradual dose reduction

If you are considering stopping a weight loss injection, discuss it with your healthcare provider, particularly if you also have type 2 diabetes. NHS information for semaglutide and tirzepatide advises speaking to a doctor before stopping and notes that a gradual reduction may sometimes be suggested.

However, the UK product information for weight loss injections does not set out a mandatory taper for everyone. A gradual dose reduction is therefore not a universal rule for stopping weight loss medication. Your prescriber can advise based on the product, current dose, side effects and individual circumstances.

When immediate discontinuation is needed

Some problems need prompt medical advice rather than a planned taper. If pancreatitis is suspected, semaglutide, tirzepatide or liraglutide should be stopped and urgently assessed; confirmed pancreatitis means the same medicine should not be restarted.

Pregnancy also requires discontinuation. Semaglutide should be stopped at least 2 months before a planned pregnancy, and tirzepatide at least 1 month beforehand; liraglutide should not be used during pregnancy. Severe allergic symptoms, persistent severe abdominal pain or severe/persistent vomiting require prompt medical assessment.

Maintaining weight after treatment ends: avoiding rapid weight regain

After weight loss injections end, maintaining weight is possible for some people, but it usually requires an active plan. The goal is not perfection: it is to prevent weight regain where possible, identify changes early and preserve results safely after stopping treatment.

Diet and eating habits

A balanced diet can help when increased hunger returns. Meals built around vegetables, fruit, whole grains, protein-rich foods and sources of healthy fats can support fullness and overall nutrition. Portion awareness also becomes more important once the medicine’s appetite suppressing effects have faded.

Because your body requires fewer calories after weight loss, returning fully to previous intake can encourage weight gain. This does not mean eating as little as possible. The aim is a sustainable pattern that provides enough nutrition while avoiding a persistent energy surplus. [13]

Physical activity and muscle mass

Regular physical activity supports weight management, cardiovascular health and muscle mass. A mixture of aerobic activity and strength training can help preserve lean muscle mass, which is important for function and body composition.

Physical activity also contributes to daily energy use and can support blood sugar control and metabolic health. Regular exercise should be realistic and progressive; an extreme programme is less useful than healthy habits that can be maintained.

Monitoring and ongoing support

Monitor weight regularly enough to notice a trend without becoming fixated on daily fluctuations. A small change away from your target weight can be a prompt to review eating, activity, sleep and other factors before more significant weight regain develops.

Ongoing support can come from a GP, specialist weight management service, dietitian or other healthcare provider. A planned follow-up is particularly useful after stopping treatment, because increased hunger and treatment weight regain can emerge gradually. Stopping weight loss injections completely is not the only possible next step. Depending on the medicine, the reason for stopping and your response so far, ongoing treatment at an appropriate maintenance dose or restarting later may be considered with medical supervision.

Continuing on a lower maintenance dose

A lower maintenance dose is only appropriate when it fits the licensed dosing options, and the prescriber judges it suitable. For tirzepatide, 5 mg, 10 mg and 15 mg once weekly are licensed maintenance doses, so some people may remain on a lower licensed maintenance dose rather than the maximum.

This should not be confused with inventing your own taper. For Wegovy, the licensed maintenance dose is 2.4 mg weekly, with 7.2 mg available for some adults with obesity when needed. Saxenda has a 3.0 mg daily maintenance dose. Any dose change should be agreed with a qualified healthcare professional.

Restarting injections later

Restarting can be considered in some cases, particularly if someone previously responded well but later began to regain weight. NICE advises considering the reason tirzepatide was stopped, the previous response and whether more diet and physical activity support would help before restarting.

If treatment stopped because of confirmed pancreatitis, tirzepatide should not be restarted. If it stopped because of other adverse effects, NICE notes that slower titration may be considered. After a longer interruption, restarting at the previous dose may not be appropriate; for semaglutide, the SmPC says a lower starting dose should be considered if multiple doses were missed.

If you previously obtained Mounjaro online, restarting still requires a fresh clinical assessment rather than simply resuming an old prescription.

Seek urgent help for severe persistent abdominal pain, symptoms of a serious allergic reaction or repeated vomiting that prevents you from keeping fluids down. A healthcare provider can also advise if stopping treatment is followed by rapid changes in appetite, weight or glucose that are difficult to manage.

How long do these injections stay in the system?

It depends on the medicine. Semaglutide has a half-life of about 1 week and can remain in the circulation for around 7 weeks after the last 2.4 mg dose. Tirzepatide has a half-life of about 5 days, while liraglutide has a half-life of about 13 hours.

Drug levels fall gradually, so noticeable effects do not necessarily stop at a precise point. Appetite, digestive factors and blood sugar effects may gradually change over time as concentrations fall.

Do side effects stop after discontinuation?

Common gastrointestinal side effects may improve as the medicine clears, but the timing varies. Nausea, vomiting, diarrhoea or constipation that remains severe or persistent should be discussed with a clinician rather than assumed to be part of stopping.

Is it necessary to stop before surgery?

Not routinely under current UK guidance. A 2025 multidisciplinary UK consensus statement recommends that people taking GLP-1 or dual GIP/GLP-1 medicines continue them through the peri-operative period, with individual aspiration-risk assessment and appropriate anaesthetic precautions. [15] The MHRA also advises patients to tell the surgical and anaesthetic team and not stop treatment without discussing it with their doctor. [1, 16]

These medicines can delay gastric emptying, which may leave stomach contents present despite fasting and increase aspiration risk during general anaesthesia or deep sedation. Always follow the instructions given by your surgical and anaesthetic team.

Is pausing treatment different from stopping completely?

Yes. A short interruption may simply mean one or more missed doses, while stopping completely means ending ongoing treatment. The restart instructions differ between products.

For example, if multiple Wegovy doses are missed, the SmPC says a reduced starting dose for re-initiation should be considered. Do not assume you can restart at the previous dose after a longer pause; ask the prescriber for medical guidance.

If you gain weight again, does that mean the treatment failed?

No. Weight regain after stopping treatment often reflects the loss of the medicine’s appetite and satiety effects together with the biological responses that follow weight loss. Many patients regain weight after stopping even when the medicine worked well during treatment.
