Using Wegovy for weight loss

Wegovy (semaglutide) is a novel medication used in the treatment of obesity. By reducing appetite and increasing the feeling of fullness, it regulates food intake and helps many people achieve significant weight loss.

Reaching a target weight is an important milestone in weight management, but it does not necessarily mean that treatment is complete. Obesity is a chronic condition, and maintaining weight loss can require ongoing support or medication. This raises an important question for people taking Wegovy: what happens when treatment stops, and how can weight regain be managed?

The truth is that after stopping Wegovy its effects will gradually diminish. The appetite may increase and cravings may return. Consequently, maintaining the desired body weight can become challenging.

For most people this means reconsidering the approach to weight management. However, it does not mean a certain weight regain. A new weight management strategy may be necessary, and discussing it with a healthcare professional is a good starting point. This will help assess potential risks and maintain health and progress.

Why people stop taking Wegovy?

Some patients may need to, while others may choose to stop Wegovy. Different factors play a role, including personal, practical, and medical ones. Here’s an overview:

Side effects, cost or a clinical reason

Wegovy does not come without potential side effects. Although most users tolerate the medicine well, side effects can sometimes lead to the decision to stop treatment [1]. The most common of these are nausea, vomiting, diarrhoea, constipation and abdominal discomfort. These symptoms are often most noticeable when starting treatment or increasing the dose. If side effects become worse, persist or are difficult to tolerate, contact your prescriber rather than adjusting or stopping treatment without medical advice.

Another factor is the cost. The treatment is a long-term commitment. When you have to buy Wegovy regularly, the cost builds up over time. Access to the medication can also sometimes be an issue. It can be influenced by changes in insurance coverage or in local prescribing services, and supply interruptions.

Additionally, your doctor may suggest stopping Wegovy for some reason. Examples include:

  • Change of health status
  • Pregnancy planning
  • Adverse reaction
  • Interaction with other medications
  • Severe and persistent abdominal pain

Reaching a goal weight

Saying that reaching a goal weight is the finish line in Wegovy treatment seems sensible. However, reaching a target weight does not eliminate the biological and environmental factors contributing to obesity, including appetite regulation, genetics, metabolism, sleep, physical activity and lifestyle.

That’s why, when possible, there should be a maintenance plan ready before discontinuing the medication that regulates all of the above-mentioned factors [2]. A solid plan should include:

  • Regular weight monitoring
  • Nutritional guidance
  • Physical activity
  • Behavioural support
  • Follow-up appointments

However, the focus should be on preserving meaningful advances in health and function, not on maintaining every kilogram of lost weight.

What happens when you stop Wegovy?

Semaglutide is the active ingredient in Wegovy. It is a glucagon-like peptide-1 (GLP-1) receptor agonist that acts on pathways involved in appetite and food intake [3]. In simple terms, it helps you feel satisfied with smaller amounts of food.

When you stop taking Wegovy, the concentrations of the medication in your body start to decline. This often results in returning to a previous (before treatment) pattern of appetite regulation. Therefore, there are no classic withdrawal symptoms like with some addiction-forming medicines [4]. But, your appetite will likely increase.

Appetite, food noise and fullness

Hunger often makes a comeback. Your meals do not provide the same feeling of fullness, and you might feel hungry sooner after you’ve eaten.

Cravings reappear in some people. Persistent thoughts of food (“food noise”) may be frustrating for individuals who previously experienced Wegovy’s positive effect on their preoccupation with eating.

All of this does not mean that you have lost your discipline or that the therapy was ineffective. It only proves that the medication was successful in regulating biological signals involved in eating.

You should make use of the new habits, established during the treatment. Regular meals, balanced portions, adequate protein and fibre, mindful eating and strategies for responding to hunger can provide structure when appetite becomes stronger.

How long does Wegovy stay in your system?

Semaglutide has a half-life of approximately one week. After the last injection, its concentration decreases gradually, and it may remain in the body for around five to seven weeks. [5] As the medication clears from your system, its appetite-suppressing effects diminish. However, the timing varies: some people notice increased hunger relatively soon, while others experience changes several weeks later.

Weight regain and the yo-yo effect

Research shows that weight regain is common after semaglutide treatment is discontinued.

In an extension of the STEP 1 clinical trial, adults without diabetes who had received semaglutide 2.4 mg alongside lifestyle intervention lost an average of 17.3% of their body weight over 68 weeks. During the following year, after both semaglutide and the structured lifestyle intervention were discontinued, participants regained an average of 11.6 percentage points of their body weight. Despite this regain, participants remained an average of 5.6% below their starting weight one year after stopping treatment. [6].

The study also found that improvements in several cardiometabolic risk factors, including blood pressure, moved back towards baseline after treatment ended. This reinforces the importance of monitoring health outcomes as well as weight.

Why does regain occur?

The diminished effects of the medication correlate with an increase in appetite. This could be due to difficulty of maintaining the energy deficit which resulted in weight loss during treatment. Other factors play a role too, including sleep, stress, and the level of physical activity.

The likelihood and extent of weight regain vary between individuals. Factors such as appetite changes, eating habits, physical activity and ongoing medical support may influence how much weight is regained.

Stopping Wegovy does not automatically result in a yo-yo effect. Some patients maintain much of their progress, while others experience more substantial regain.

While willpower is important, it is not enough for obesity management. So, instead of blaming the individual, it is better to recognize that weight maintenance requires ongoing treatment and support.

Is it safe to stop Wegovy suddenly?

While Wegovy is not associated with a drug-withdrawal syndrome, the NHS still suggests consulting your doctor before discontinuing its use [7]. For some patients, they may recommend gradually reducing the dose instead of stopping suddenly. For example, in individuals with diabetes, or a pre-existing treatment plan for other conditions.

The bottom line is: There are no reasons for gradually reducing the dose due to physical or psychological withdrawal symptoms. The only reason for a gradual and individualised approach is to better manage the transition and monitor appetite, weight, blood glucose, and other symptoms.

How to stop Wegovy and not regain weight?

A maintenance strategy combined with structured support and starting before the final dose of the medication is best way prevent or limit weight regain. Here’s an overview:

Eating and activity habits to keep

With or without Wegovy, or some Wegovy alternative, lifestyle changes are essential to weight management [8]. The focus should always be on establishing routines that support adequate nutrition, manage hunger, and protect physical function:

  1. Balanced diet: A highly restrictive diet can be difficult to maintain over time. Instead, focus on a sustainable eating pattern that includes vegetables, fruit, wholegrains, healthy fats and adequate protein.
  2. Prioritise lean proteins: Foods high in protein but low in harmful fats, such as fish, eggs, poultry, beans, lentils, tofu and lower-fat dairy products are a great choice. Adequate protein intake will support satiety and preserve muscle during weight management.
  3. Regular physical activity: Keep yourself moving every day. Walking, cycling, swimming, and other activities support weight management and cardiovascular health.
  4. Strength training: Resistance exercise helps preserve or build muscle, particularly after significant weight loss. Maintaining lean mass supports strength, physical function and long-term health.
  5. Manage hunger realistically: The best strategy to keep hunger under control is to sleep enough, eat slowly, and pay attention to fullness. Meal planning is another great idea. Make sure you have nutritious foods at hand in case you need a snack. Try and identify situations that trigger unplanned eating so you can avoid them or control them better.
  6. Monitor progress without becoming preoccupied: When you track weight regularly, you can notice downward or upward trends. Still, fluctuations are normal. For example, the scale will always show a bigger number in the evening than in the morning. That’s why other factors require tracking too, such as waist circumference, fitness, strength, and relevant health markers.

Maintenance dose, restart or switch

Sometimes, continued medication treatment is appropriate. That’s why we should not think of obesity medications as a temporary intervention that’s finished once we reach our goal weight.

There is such a thing as a maintenance dose of the medication. However, its suitability depends on the licensed indication, current prescribing guidance, treatment response, tolerability, and the individual's health needs. This is a decision for your healthcare provider to make.

Restarting Wegovy after an interruption is potentially an option, but it should only be considered after consulting your prescriber. The appropriate restart dose depends on different factors [9]. For example, how long treatment has been interrupted and whether restarting at a previous dose could increase gastrointestinal side effects. Again, your doctor should determine the safest approach.

Another possibility is switching to a different weight loss medication, such as tirzepatide (Mounjaro). Tirzepatide acts on both GIP and GLP-1 receptors, whereas semaglutide acts on GLP-1 receptors only. However, the Wegovy vs Mounjarodilemma is up to your prescriber to resolve. What you need to understand is that switching does not guarantee better maintenance, and the medicines should not be combined or substituted without an appropriate clinical plan.

Key takeaways

  • Appetite and cravings may return gradually after stopping Wegovy.
  • Weight regain is common, but the amount varies between individuals.
  • Sustainable eating habits, physical activity and ongoing support can help maintain progress.
  • Always discuss stopping, restarting or switching medication with your prescriber.

Frequently asked questions about stopping Wegovy

Do I have to take Wegovy forever?

Obesity requires long-term treatment. However, the duration differs among individuals based on the treatment response, personal health needs and preferences, and the doctor’s advice. Some people benefit from continuing medication to maintain weight loss. Others can make it work with a good maintenance plan.

What happens if I stop Wegovy for a week?

Missing one weekly injection does not mean that semaglutide immediately leaves your body. Its concentration declines gradually, although changes in appetite and treatment effects vary between individuals.

If you miss a Wegovy injection, take it as soon as possible if no more than five days have passed since the scheduled dose. If more than five days have passed, skip that dose and take the next injection on your usual scheduled day. Do not take a double dose.

If you have missed multiple doses, contact your prescriber before restarting, as restarting at a lower dose may be appropriate to reduce gastrointestinal side effects.

Should I stop Wegovy if I want to get pregnant?

Yes. Semaglutide should not be used during pregnancy. If you are planning a pregnancy, the UK product information recommends discontinuing Wegovy at least two months beforehand because of its long half-life [10]. If pregnancy occurs while taking it, contact your doctor without delay.

Can I pause Wegovy instead of stopping?

A temporary interruption may be necessary because of illness, Wegovy side effects, access difficulties or other circumstances. Make sure to discuss any break longer than one dose with your healthcare provider.  Restarting may require Wegovy dose adjustment. Appetite and weight may also change during the interruption.

Can I come off Wegovy if I have diabetes?

This is not a decision you should make yourself. Although Wegovy is prescribed for weight management, semaglutide products are also used in diabetes care. Therefore, stopping treatment may affect blood glucose. You should have your doctor review all current medicines, glucose levels, and whether alternative treatment is needed before deciding to stop Wegovy.

Can I switch to Mounjaro after stopping Wegovy?

Switching to Mounjaro (tirzepatide) may be appropriate for some patients. However, this is another decision you need to make together with your healthcare provider. The eligibility depends on the indication, previous response, and the prescribing guidance in your country.
