Wegovy Pill Dosage: Everything You Need to Know Before Starting

The Wegovy pill dosage schedule

The weight loss treatment with the Wegovy pill involves once-daily oral semaglutide [2]. The Wegovy tablets are taken once daily and are available in four strengths: 1.5 mg, 4 mg, 9 mg and 25 mg. The dosing schedule starts with a low strength and progresses towards the recommended maintenance dose. The prescribed starting dose is 1.5 mg once daily, followed by 4 mg, 9 mg and finally 25 mg, with at least one month at each stage. Patients should not change the dose of Wegovy themselves.

According to the current UK Summary of Product Characteristics, each of the four Wegovy dosage stages lasts a minimum of one month.

Wegovy tablets are taken once daily, with the dose gradually increased over the first four months of treatment. Starting at a low dose and increasing it gradually is intended to imprive tolerability, particularly by reducing gastrointestinal side effects during dose escalation.

The maximum recommended dose is 25 mg once daily. Take only one Wegovy tablet per day — you should not combine several lower-strength tablets to achieve a higher dose same day.

According to the prescribing information, patients may remain at the previous strength if necessary. This means it is possible to adjust the planned dose of Wegovy according to tolerability. There is no need to automatically increase it on a fixed date regardless of symptoms.

However, a qualified healthcare professional should determine the optimal dose for each individual’s circumstances.

Oral Wegovy (semaglutide) is a popular alternative to weekly injections. The use of these tablets in the UK is authorized for adults with obesity or overweight individuals with at least one weight-related health condition (alongside a reduced-calorie diet and increased physical activity) [1].

How to take Wegovy pill correctly?

The Wegovy injection differs from the oral formulation. The main differences are in the method of administration (subcutaneously, once a week) and absorption (with or without food). Unlike the injection, the tablet is taken daily and under specific conditions designed to support absorption.

The current UK instructions for Wegovy pills are:

  1. Take one tablet once a day.
  2. Take it in the morning after fasting for at least eight hours.
  3. Swallow the tablet whole with a small amount of water (up to 120 ml).
  4. Do not split, crush, chew, or dissolve the tablet.
  5. Wait at least thirty minutes before eating, drinking, or taking other oral medications.

These instructions are important because oral semaglutide is absorbed predominantly in the stomach, and its absorption is affected by food and the amount of water taken with the tablet.

Why does taking it on an empty stomach matter?

Taking the tablet on an empty stomach and waiting at least thirty minutes before eating, drinking, or taking other medications increases semaglutide absorption [3].

This is one of the most important differences in the Wegovy pill vs injection discussion. The injection does not have the same fasting requirement because it bypasses the digestive system.

However, individuals who regularly take other oral medications in the morning may need to reorganize their routine.

How to build the routine into your morning dosing schedule?

Consistency makes daily treatment easier. It’s a good idea to associate the tablet with some other regular morning activity, or to keep the medication where you usually prepare for the day. A practical time to take the tablet is in the morning after the required fasting period. This usually allows enough time to absorb the medication before breakfast.

*Do not take another dose of Wegovy if you are unsure whether you already took a tablet.

What happens at each dose stage?

Wegovy treatment typically involves gradual dosage escalation due to adverse gastrointestinal effects. Nausea, vomiting, diarrhoea, constipation and abdominal discomfort are among the gastrointestinal symptoms that can occur [4].

The gradual increase gives the body opportunity to adjust and enables the prescriber enough time to assess whether the current strength is well-tolerated before moving to the next stage. [5]

1.5 mg: Wegovy pill starting dose

This is the lowest available Wegovy tablet strength, and it is used as a starting dose during the adjustment period.

The low dose also allows the assessment of both the positive and negative effects, such as appetite regulation, gastrointestinal function, nausea, bowel habits, etc.

Experiencing little or no weight loss in this early stage of treatment does not mean the dose should immediately be increased. The initial stage is primarily about establishing tolerability and following the prescribed titration plan.

4 mg: the second dose for Wegovy pill titration

After at least one month at 1.5 mg, the next stage is 4 mg once a day. Some patients may notice a stronger effect on appetite at this stage, while others may experience more gastrointestinal symptoms. The response varies substantially between individuals.

If significant or troublesome symptoms occur, do not change or stop the dose yourself; contact your healthcare professional, who can decide whether the dose should be maintained, adjusted or treatment discontinued.

9 mg: the third dose for further Wegovy pill dose escalation

The third stage is 9 mg once daily, again after a minimum of one month at the previous dose. Monitoring remains crucial. A patient who has tolerated 1.5 mg and 4 mg well may still notice gastrointestinal effects after the increase to 9 mg.

There should be no rush to reach the highest dose as quickly as possible. Finding a dose that provides an appropriate therapeutic response while remaining tolerable is much more beneficial in the long run.

Wegovy pill 25mg dose as the maintenance dose

The Wegovy pill maintenance dose is 25 mg once daily. It is the highest recommended daily tablet dose.

The prescribing information for Wegovy pill in the UK recommends escalation towards 25 mg. However, it also states that the treatment may be maintained at the previous level if necessary. (Therefore, it would not be medically accurate to define the highest dose as the best dosage for weight loss.)

If a person is experiencing significant gastrointestinal symptoms, a doctor may decide to delay escalation and maintain the current strength longer.

Therefore, the maintenance dosage is the recommended long-term target. It is not a goal the patients should force themselves to reach, regardless of tolerability.

What if moving to the next dose feels too hard?

Nausea, vomiting and diarrhoea are common Wegovy pill side effects. They can be particularly noticeable at the beginning and with dose increases. Dehydration can also become a problem when diarrhoea and vomiting are present.

Always consult a doctor when moving to the next dose feels difficult. Depending on the circumstances, they may recommend remaining at the previous strength for additional weeks.

With this in mind, you should not increase or reduce the dose of Wegovy yourself, for whatever reason. Also, seek medical advice if vomiting or diarrhoea is severe or persistent, or if you cannot maintain adequate hydration. Severe symptoms, persistent abdominal pain also requires urgent medical assessment because acute pancreatitis has been reported with GLP-1 receptor agonists [6].

What to do if you miss a Wegovy pill?

The instructions for a missed dose are straightforward: skip the missed tablet and take the next tablet on the following day [7]. Do not try to compensate the scheduled dose by taking two pills!

This is different from the instructions for the Wegovy injection, for which a missed weekly dose may be administered within five days, depending on how long it has been since the missed dose.

However, a single missed dose is different from missing several consecutive doses. When you stop the treatment for an extended period, consult a medical professional before restarting.  In such cases, a lower starting dose may sometimes be appropriate.

How does the Wegovy pill dose compare to the injection?

Both formulations contain semaglutide. However, tablet and injectable doses are not directly interchangeable milligram-for-milligram. That’s because oral absorption is different from subcutaneous administration, and the strengths and dosing frequencies are different:

  • The oral formulation is taken once daily. The tablets are available in 1.5 mg, 4 mg, 9 mg, and 25 mg dosage strengths.
  • The Wegovy injection is administered once weekly [8]. The available injectable strengths include 0.25 mg, 0.5 mg, 1 mg, 1.7 mg, 2.4 mg, and 7.2 mg (for appropriate patients in the UK).

*The Wegovy oral dose 25 mg is not equivalent to 25 mg injected as the two routes have different pharmacokinetic properties.

How to switch from the injection to the pill?

Switching formulations should only be done under medical supervision. The appropriate tablet strength differs among individuals and depends on the previous semaglutide treatment.

For example, the UK product information specifically addresses switching from a 2.4 mg once-weekly semaglutide injection to oral semaglutide [9]. Patients receiving 2.4 mg once weekly can transition to 25 mg tablets once daily (with the tablets started one week after the last injection).

However, other clinical circumstances still require individual assessment.

Which pill dose matches your injection dose?

There is no simple formula for converting an injectable milligram dose into an equivalent oral milligram dose. That’s why you should always let your doctor determine the appropriate dosage and not rely on the Wegovy online conversion chart.

The approved UK transition (previously mentioned), from 2.4 mg weekly Wegovy injection to 25 mg oral semaglutide once daily, is a specific prescribing recommendation, not a general conversion rule for every possible injectable dose.

Do you need a break before switching?

For patients transitioning from 2.4 mg once-a-week injectable semaglutide, the UK product information states that oral tablets can be started one week after the last injection.

The half-life of semaglutide keeps the timing relevant. The medication remains in the circulation for several weeks after the last dose. Therefore, starting treatment at an inappropriate interval could affect overall exposure.

Do not independently shorten the interval between the last injection and first tablet. Base the timing of the next scheduled dose on your prescribed transition plan.

How long will you need to take the Wegovy pill?

Wegovy, together with appropriate dietary and physical activity measures, is intended for long-term weight management and maintenance [10].

That’s why the duration of Wegovy treatment is individual. Your doctor will consider your response, tolerability, medical history, and treatment goals before deciding if continued treatment remains beneficial.

Always keep in mind that weight management is a long-term treatment, and maintaining weight reduction may require ongoing lifestyle support.

Wegovy pill dosage and weight loss: what to expect?

A higher dose does not equal faster results. The dose escalation is not meant to slow down the weight loss, but to establish tolerability to the medication.

Clinical studies have demonstrated substantial average weight reduction with oral semaglutide (with varying individual responses).

In the OASIS 4 trial, 25 mg oral semaglutide produced a mean weight loss of 13.6% at week 64. In an analysis assuming continued treatment without additional anti-obesity medication or bariatric surgery, the estimated weight loss was 16.6% [11]. However, trial averages should never be interpreted as a guarantee for an individual patient.

The purpose of treatment is to help eligible patients reduce excess body weight and support long-term weight management.

A slower response does not necessarily mean the treatment is ineffective. Therefore, increasing the dose of Wegovy earlier than recommended is not a safe way to try to accelerate results.

FAQs about Wegovy pill dosage

Can I take the Wegovy pill at night instead of the morning?

Theoretically, this is possible. However, the tablet should be taken after a fasting period of at least eight hours, followed by at least 30 minutes before eating, drinking or taking other oral medicines. Morning administration is therefore usually the most practical option, but the key requirement is following the prescribed fasting and post-dose interval..

What if I eat or drink too soon after taking the pill?

The prescribing information states that waiting less than thirty minutes can reduce semaglutide absorption. Therefore, if you eat or drink too soon, the amount of medicine absorbed may be reduced.

Can I crush or split the Wegovy pill?

No. Wegovy pills should be swallowed whole. The UK product information advises that tablets should not be split, crushed or chewed. This applies to all tablet strengths.

Can I increase my dose faster than the schedule?

No! Do not accelerate the Wegovy pill dosing schedule yourself. The recommended titration allows at least one month at each strength. Moving more quickly may increase the risk of gastrointestinal intolerance.

Do you have to reach 25mg?

The individual’s maximum tolerable dose is the maintenance dose.

The 25 mg dose is the recommended long-term target for oral Wegovy. However, the prescribing information allows treatment to remain at the previous dose level if necessary.

Doctors may delay escalation when tolerability is an issue and determine whether remaining at a lower dose provides an acceptable balance between benefit and side effects. A carefully supervised Wegovy treatment plan will balance efficacy, tolerability, adherence, and long-term health goals.
